2. Complete Rebate Form
3. Mail completed Rebate Form along with your sales receipt of the qualifying purchase.

Your submission must contain to be valid:
- A completed redemption form (download)
- Your sales receipt from the qualifying purchase (receipt must show purchase paid in full)
Documents must be received no later than November 30, 2012
- Purchase must be made at an authorized Pacific Energy Fireplace Products Dealer
- Allow 6 - 8 weeks for delivery
- Valid the USA and Canada Only
- Not Valid with any other offer
Terms and Limitations
Purchase items(s) between August 1, 2012 and October 31, 2012 qualify for all rebates.
Offer good only on items listed above, one rebate per product purchased and per household. Purchases made before or after these dates will not be eligible.
Promotion Restrictions
Rebate must be paid to the consumer (homeowner). Offer is not valid in conjunction with any other offers.
Claim Procedures
Claims must be submitted via mail-in rebate. Rebate certificates may not be altered. Incomplete certificates will not be honored.
Please keep copies of all materials submitted for your own records.
